Community Health Assessment is a core function of public health agencies and a fundamental tool of public health practice.  Its aim is to describe the health of the community by presenting information on health status and other studies of current local health problems, community health needs, health care and community resources.  Multiple community partners assist with this process.  View the following links for related information.

In addition to these two priorities, ongoing work will continue in various areas such as diabetes prevention, obesity reduction, asthma self-management, immunizations, lead poisoning prevention, communicable disease tracking, and breastfeeding education.
