Long Term Home Health Program

 The Saratoga County Long Term Home Health Care Program (SCLTHHCP) is one of many programs offered by Saratoga County Public Health.  SCLTHHCP is certified and overseen by the New York State Department of Health.

 The Long Term Home Health Care Program offers a variety of health services to individuals with chronic, debilitating illnesses who are no longer able to manage independently in the community.

 A nurse from Public Health and a caseworker from the Department of Social Services would complete an assessment to determine the type and frequency of services needed to safely maintain an individual in his/her own place of residence.  An individual must need at least one of the required waivered **LTHHCP services to be admitted to the LTHHCP.
