Your kids need health care.  Now you can afford it!

Child Health Plus is an insurance plan for children.  This plan is available through dozens of providers throughout the state.

The following services will be paid for by Child Health Plus:

  • Well-child care

  • Physical examinations

  • Immunizations

  • Diagnosis and treatment of illness and injury

  • X-rays and lab tests

  • Outpatient surgery

  • Emergency care

  • Prescription and non-prescription drugs if ordered

  • Inpatient hospital medical or surgical care

  • Short-term therapeutic outpatient services (chemotherapy, hemodialysis)

  • Limited inpatient and outpatient treatment for alcoholism and substance abuse, and mental health

  • Dental and vision care

  • Speech and hearing

  • Durable medical equipment

  • Emergency ambulance transportation to a hospital

  • Hospice

Eligibility Requirements:

Children under the age of 19 who are not eligible for Medicaid and have limited or no health insurance may be eligible for Child Health Plus.  Even if your family income is high, you can enroll in Child Health Plus.

There are NO CO-PAYMENTS for services or benefits received!  Depending on your gross family income, you may have to pay a family contribution to enroll in Child Health Plus.

Whether a child qualifies for Child Health Plus A or Child Health Plus B depends on gross family income.  Children who are not eligible for Child Health Plus A can enroll in Child Health Plus B if they don’t already have health insurance and are not eligible for coverage under the public employees’ state health benefits plan.

Where do your kids go for health care?

Once you select an insurer, you will be given a list of providers near you.  The provider may be a single doctor, group practice of several doctors, or community health center.  If it is a group practice, you may be asked to choose a doctor for your child.  Either way, you will be getting the personal health care that your child deserves!

Can you qualify even if you’ve been turned down for Public Assistance programs in the past?

YES!  Child Health Plus is available to all New York State residents who meet all other eligibility criteria, regardless of their income!  Even if you are working, your children may be eligible.  Call 1-800-698-4KIDS (1-800-698-4543) for more information
