Saratoga County Department of Health Facilitates the New York State CYSHCN Program for Saratoga County residents. The Program’s mission is to ensure all children and youth with special healthcare needs have adequate health insurance, services, and community resources and to empower families and youth to navigate community systems of care. The program provides families with assistance accessing community resources and care, eliminates gaps and barriers to accessing care and resources, and assists youth with transitioning from pediatric to adult health care systems.

Eligibility for CYSHCN is a child or youth who:

  • Is age birth to 21 years of age
  • Is a resident of Saratoga County.
  • Has or might have a disability or chronic health condition.

CYSHCN conditions include:
ADD or ADHD, allergies, anxiety, arthritis/joint problem, asthma, Autism or Autism Spectrum Disorder, behavioral/conduct problem, blood disorder, cerebral palsy, cystic fibrosis, depression, developmental delay, diabetes, down syndrome, epilepsy/seizure disorder, genetic or inherited condition, head injury, hearing problem, heart problem, intellectual disability, learning difficulty, migraines, mental health condition, muscular dystrophy, sickle cell anemia, speech or language disorder, substance use disorder, Tourette syndrome, and vision problems.

*The program is not limited to these conditions.

  • CYSHCN is a New York State Department of Health initiative which provides families with assistance accessing community resources and care, eliminates gaps and barriers to accessing care and resources, and assists youth with transitioning from pediatric to adult health care systems.
  • The New York State Multiple Systems Navigator allows users to access helpful health, education, human service, and disability information on one user-friendly website. The Navigator is built for youth, parents, family members, and caregivers that rely on supports from multiple child and family serving systems.
